As a participating organization with Centers for Medicare and Medicaid Services (CMS) the Arkansas Palliative Care Dementia GUIDE Program is committed to enhancing the quality of life for those affected by dementia by offering specialized care and resources designed to meet their unique needs, as well as added support for caregivers.

Your GUIDE for care and support

The Arkansas Palliative Care Dementia Guide Program is a specialized initiative in conjunction between CMS and Arkansas Palliative Care that is designed to provide comprehensive support and care coordination for individuals with dementia who are covered under Medicare Part A and B.

The GUIDE (Guiding on Improved Dementia Experience) Model program aims to enhance the quality of life for patients and their families by offering personalized care plans and expert guidance through the complexities of dementia care.

A range of services from an Interdisciplinary Care Team

  • Comprehensive Care Coordination – Our Clinical Care Navigators, backed by an entire interdisciplinary team, will work closely with patients, caregivers, and healthcare providers to develop personalized care plans and ensure seamless coordination of medical, social, and support services.
  • Specialized Dementia Care Planning – We offer expert guidance on symptom management, behavioral interventions, and advance care planning specifically geared toward dementia care.
  • Access to Palliative Care – Patients who qualify may enroll for additional Arkansas Palliative care services aimed at improving symptom assessment, enhancing quality of life, and providing emotional support.
  • Educational Resources – We provide educational materials and resources to empower caregivers with the knowledge and tools needed to navigate dementia care effectively.
  • Telehealth Benefit Enhancement – Available support for certain situations and remote locations.
  • Up to $2,500 Annual Allowance for Respite Care – In addition to the comprehensive services offered by the Arkansas Palliative Care Dementia GUIDE Program, eligible patients and families can benefit from up to $2,500 in annual reimbursement for in-home, adult day center, and facility-based respite care. This reimbursement is designed to provide much-needed relief to caregivers and families, allowing them to take necessary breaks while ensuring continued support and safety for the patient.

The Arkansas Palliative Care Dementia GUIDE Program is covered by Medicare for eligible patients. There are no additional costs or fees associated with participation in this program beyond standard Medicare coverage.

Who provides care under this program?

Care providers in the GUIDE Model (“Participants”) are a limited number of care providers who have applied to and been accepted by CMS to provide care through this innovative program. The Arkansas Palliative Care Dementia GUIDE Program care team members are Certified Dementia Care Specialists comprised of physicians, nurse practitioners, and other allied health professionals/partner organizations dedicated to delivering compassionate and effective care for dementia patients and their families who qualify.

Who is eligible for the Dementia GUIDE program?

Persons who are enrolled in Medicare Part A and B as their primary payer and have mild, moderate, or severe dementia as a confirmed diagnosis or related cognitive impairments are eligible for the CMS GUIDE Program.

Arkansas Palliative Care is a CMS-approved participating provider of the GUIDE program to individuals who qualify and reside in the Arkansas Palliative Care service area. Individuals who are long-term nursing home residents, enrolled in a Medicare Advantage Plan, PACE, or hospice program are not eligible.

Support & comfort beyond dementia care

The Arkansas Palliative Care Dementia GUIDE Program stands out for its patient-centered approach, specialized expertise in dementia care, and commitment to improving the quality of life for individuals with dementia and their families. WE emphasize personalized care planning, education, and support to address the diverse needs of our patients.

Patients enrolled in the Arkansas Palliative Care Dementia GUIDE Program not only receive the benefits of this program, but also receive the added support of Arkansas Palliative Care. This includes support and comfort beyond treatment such as:

Symptom assessment.

Understanding treatment options.

Dealing with co-morbidities.

Navigating the complicated healthcare system.

Identifying needs and providing direction for additional resources for spiritual or emotional care.
